This may be a dumb question but are navigation map updates done over the air automatically, assuming I’m fully subscribed to all of MB’s services (which I am)? I’ve owned my car for two years and never noticed a map needs updating message. I also just briefly browsed the services only and could find no map update section, at least from my cursory browsing.

Look at your mercedes me app to see what updates have been done. You can always go to the web site, download the latest to a thumb drive, and install yourself using the media cable supplied with the car

Go to maps and look for version info. Tell us the version you have and we can compare that way, usually it is the best to manually update by downloading to a storage media then plugging it in the car. That is preferred as you don't have to wait for OTA which is staged and depends on the reception. Manual updates you are in control instead of the car controlling you. Speech data can only be updated via manual updates as well.
If you go to MBUSA website on your PC, then navigate to the map update section, then click to update regions manually, you can check to see what version of different regions of the map you have and what is available. In the screenshot below, Georgia is up to date as I did that manually last week, but Oklahoma, Arkansas and so on, is out of date. The Oklahoma region is at Version 19 and Version 23 is available for download.
As for your question on should maps update over the air, I've only had that happen one time, every other time I've done them manually as Crab suggested.

Yeah I was hoping these updates were done OTA but apparently not. There’s nothing in the App OTA updates that shows past map updates for me.
It will eventually get to you via OTA but it will take a while. The speech data however you have to manually update it, OTA won't contain speech data updates.
