May sound too simple, and maybe I missed it somewhere, but did you pull oil fill cap and look to see if you have a “frothy” looking oil on the chain? Your oil will get a frothy look if it is mixing with coolant.

Hey thank you for your comment. We did check that and it looked good. Nothing under the coolant cap either. Im not sure how, but seemed like the only tell was loosing coolant and it showed up in the oil sample. Same with taking some oil and letting it sit for awhile showed separation of the oil and coolant.

I know this is an old thread, but I was a little surprised that no one mentioned doing a block test for exhaust gases in the coolant reservoir. Very quick way to check for a blown head gasket or cracked head.
