My 2002 C320 has leather inserts which are fine but I would like better quality leather. If I order "Premium" leather will it be a different quality leather or merely covering more surfaces?

The latter.

Some Mercedes have the option for "Nappa" leather - but I don't think the C-class (other than the C32) has that option.

The Nappa on the C32 is definitely softer and thinner. It is definitely more "luxurious" at the expense of being more delicate.

I don't think the Premium leather is worth the money. It only adds leather to the bolster strips. Even the Premium leather still has cloth sections

how many cows?

Okay, on a Coupe w/ MB leather, how much of the seat is leather? My Coupe came with cloth because of the shortage of leather due to Mad Cow, and a local shop put leather in it. I was looking at a pic in another thread of a leather interior, and it looked like the seat backs were plastic-mine are leather wrapped? The only fabric I have is on the door pockets and the cargo cover-everything else is leather. Dealer tried to charge me what it cost them ($2,200-I have the invoice), but I eventually just paid the same upcharge as if it were factory ($1,200). Just curious...

The MB-Tex (?) which comes with the inserts (sedans) is really very nice IMO, and I find it hard to tell the difference between it and real leather. The most noticeable difference is in the Airbag covers on the doors. The leather looks looser, and I think it's pleated, where the MB-Tex is more taut.

if you like your car to smell leather for a long long time, premium is worth it. other than that it's not. unlike BMW premium from MB are not Nappa leather. Connoly (sp?) leather from Jaguar and RR or Bently is still the best leather available for cars.

As far as I know thinner does not mean better leather, just the opposit. At least in leather furniture

Most of the seatback on my coupe is a hard plastic shell. The door panels have leather where yours have cloth. The seating surfaces, the top and sides of the backrest, and part of the front and sides of each bench are leather, but there is cloth on the lower front and sides of the bench. Somebody on another thread suggested that leather on the lower part of the bench would stretch.

I'd say you got a good deal, and your interior will certainly be distinctive.
