Is this what I think it is?

Ticking...

2013 P31, manufactured April 2013. Just started today, haven't driven in maybe a day and a half. Started up to this, sound fluctuates with rpm...

What am I in for, and what is it going to cost?

Sounds normal to me, apart from the whiring sound which could be a PS pump or idler pulley somewhere.

You can hear the ticking 10 feet from the car.

Kinda loud

ETA: completely quite again when warm. Changed the oil about 2.5k miles ago, it was done at an MB dealership.

Much more audible in that video. Its normal for the lifters to drain down when it hasn't been driven for a while. Mine only does it when left for 2+ weeks. What oil is in it?

I've never heard this sound even after a week of sitting before.

How many miles?

Me and a friend that runs a Mercedes garage in the UK have the same experience with the M156.

Pull the rocker covers and see what the condition of the cam lobes are as well.

I was thinking of going with Liqui-Moly, plus a couple of the additives ive seen suggested here like Cera Tec next oil change in about another 2k mi. If this noise keeps happening on start up, I'll do it this week, actually.

Not a bad idea. If you have a blocked lifter that should help. I run forte oil flush at every oil change.

That's the only oil I use for my high performance cars now
