1) You can't; the law says it has to be on by default (in Europe at least)
2) You can't, but what you can do is press the start button without your foot on the brake (i think you have to do this twice), and then you can select your driving mode before putting your foot on the brake and starting the car up, so it'll start in loud mode. Not quite the same, but an option at least.
My solution is set up Indivudual mode to your liking. Now its just second nbature to get in the car. press the start button, hut up on the drive select, and put the car in drive.

My solution is the same as Yours and it does become second nature.
I forgot to say that driving with the engine in sport+ mode (not sure about sport), whether set that way by sport+ or individual, stops the auto start-stop from coming on.

Opening the exhaust valve before turning on the engine is kinda useless because the car automatically opens the valves for the initial startup growl.
