My E350 gets significantly better gas mileage than that in Eco, and Sports mode. Spirited driving gets me around 18mpg, and I usually get around 24. If I'm on the highway then I've gotten up to 30 mpg (7l/100km)

So time for spark plugs and maybe spark coils… either that or i need to enable full manual mode and take it for a good rip roaring spin and maybe it will get better.

has anyone had any other potential maintenance items to chase on that?

Camshaft solenoids, and sensors. The factory ones leak bad, on all w212's until the revised part came out on 2016.

I dislike ECO mode and regardless of the car I drive in I instantly turn it off .If mileage was my main concern I would buy a diesel car

Not really true. I just completed a 1300 mile weekend trip. At 70mph on cruise control, (E mode) the engine turns 1956-2000 rpm for hours on end. 34.6 mpg.

Essentially the same engine in my C320 turns 2600 rpm in exactly the same conditions, barely cracking 31 mpg. I think it's all in the gearing, 5-spd in the W203 vs 7-spd in the W212.

Have you ever used injector cleaner in your fuel? Dirty injectors can really hurt fuel mileage.

How many miles since your last "tune-up"? I will look at the basics:
1 - Miles on the air filter, spark plugs ?
2 - Check for codes related to the engine
3 - Reset the tranny, and retrain it?
4 - Coils should trigger codes (not necessarily cluster messages) when they have issues.

Mine has @70K+ miles, spark plugs/air filter/tranny+engine oil @60K, no codes, no leaks around the engine and the mileage is between 22->30MPG with some spirited driving here and there (S when alone, E with company).
