when they fail. Very rare...

They are not a maintenance item. You replace them when they fail, a check engine light and glow plug light will illuminate when the system detects one is failed.

Yep, but not because of the glow plugs. Glow plugs only prewarm the chamber on startup.
From various threads I believe the reason for the noise is wear of the injectors. The quality of the atomisation has a huge impact on combustion.
Not much to do apart from changing them and that is $500+ per injector.

That's why I wont buy another diesel. I grit my teeth everytime I accelerated from stop because of all the noise. Still quiet at highway speeds.

Glow plugs won't help with any noise. Lately I've seen some timing chains stretching out causing some noise, this on the v6 om642, usually higher miles.

so I need to change timing chains? I thought timing chains can last for 200,000km or more

No no,.... dont change timing chains.... this is not the same noise.

I did not say that. I'd have to hear the noise and there is also a specific test for the chain to see if it's stretched beyond specification.
