Only 12 mpg
I have a 2001 s430 with 190k miles I have replaced the air filters and spark plugs and cleaned the maf. I mostly drive just to work in town ( 5 mile round trip) and barely go past 3k rpm but I only get 12 mpg what else could be wrong?

Plugged cats
Worn Cam
Slipping transmission
Dragging brake
Bad alignment
Bent or worn suspension component
Cleaning the MAF rarely works well. But if the engine runs OK, it might have worked.
I would start with a general inspection on everything. How are the tires? Is one brake really hot when you're done driving? How's the tire wear?
If all that's OK, check compression, and if that's OK, change the MAF...

P0110 code will be often preceded by the Check Engine Light coming on the vehicle dashboard display. The vehicle will most often not run well, idle poorly, accelerate erratically, run rich and backfire because the IAT sensor and throttle position sensor are not electrically synchronized together.

There are voltage measurements involving both. Depending on the severity of the condition, the diagnosis and repair for error code P0110 can be meticulous, if not difficult, and would definitely require advanced tools and of course, skills. If I were you, I wouldn't hesitate to take your vehicle to your dealer or to a qualified mechanic.
