Update: 93 octane made no difference than 91.
Now aside from my OP re: stoplight idle vibration/noise, the more irritating thing is the 'groaning' rumbling vibration that also comes thru the steering wheel which occurs in typical block by block city driving at 30mph/50kmh (1100 rpm) with a slight step on the gas pedal to maintain speed with traffic. Every time you slightly step on the pedal, the grumbling occurs. You think I could get it to happen at the dealer, not a chance, so pissed off.
Anyway, got another dealer app't since it's been happening the day after the 1st visit last Thurs. Any ideas folks? My 2019 E450 wagon didn't have this issue.

In your garage with the motor off, rock the steering wheel. Generally you can hear worn bushings, if any.
Have a proven Indy suspension shop inspect the front suspension system.

Went to the dealer today and he suspects the engine mounts. He has a 2018 S560 in for the mounts due to what seems to be the same prob. So I'll wait to see if that car drives better with the new mounts.
Also ordered a new battery since my morning voltage before starting was 12.30 volts.

Well I'm still waiting for the dealer to replace the engine mounts. He has replaced the trans mount but taking a day and a half seems long to replace engine mounts. Does that seem right to you?

We have an E Class cabriolet and a S Class. At only 20,000 miles the E Class needed new engine mounts. The car would shake like crazy at idle. It was replaced under warranty. They also did the transmission mount. The mechanic said he sees it a lot, ever since “auto stop/start” came along.
I have since programmed Auto Stop/Start to be turned off by default when starting. Did this on both the E Class and my S Class. I believe Auto Stop/Start is the biggest scam ever. It is used for auto manufacturers to get a lower emissions scores, which they need to comply with legislation. However, the savings and benefits to the consumer is nothing, maybe $50 a year in petrol? I don’t see how these vehicles are designed to start 50-100 times a day without some significant wear and maintenance costs. The “emission savings” is also redundant given all the old worn parts are now in land fills.

There is a "free" approach which is to unplug one of the fuses but that affects more than the start-stop, it affects the alternator which can lead to damages in the long term (your car will rev higher on idle too) hence why it is "free" and not free. (I haven't find the post talking about which specific fuse yet and it too differ from different model of MBs, so probably don't recommend this either way)
Another approach would be just muscle memory when starting the car and hitting the start-stop button or just switch to individual with the setting to auto-stop disabled chosen.
Edit: Forgot this method too exist, although I don't know if it safe but I will share it here anyways but please proceed with caution:

Last but not least, disconnect or alter the auxiliary battery (highly not recommended)

Now, every time you start your car, ECO (or Auto Stop/Start) is off. If you want it on (for some reason), simply push the ECO button again. Press again to switch off.
Yes, I think its expensive for what it is (mine on eBay was approx USD$120), but its one less hassle to think about. And I hate auto stop/start that much.
I’ve noticed in some interior photos that the ECO (Auto Start/Stop) button is heavily worn. I’m guessing they press it every time they start their car to switch it off.

Dealer finally got the engine and trans mounts in another 2018 560 and I went for a test drive in it 'on their lot' only. The steering wheel vibration was minimal. However since we didn't actually do a city road test, I'll never know if it solved the prob until I check back with the Ser Adv and see if the customer was satisfied.
Then while I was there, the mechanic went with me to see if I could get my vibration issue to display itself with me driving. NOT A CHANCE! I'm so frustrated cause all the way there, the problem was more than evident.
One noticeable observation, in the last couple weeks waiting for the dealer to do the repairs, I have discovered that shutting the ECO off, the problem almost disappears! What the heck is going on here. As I pressed the ECO switch off, i could sense a slight transmission adjustment.Totally baffled.
Then while I was there, the mechanic went with me to see if I could get my vibration issue to display itself with me driving. NOT A CHANCE! I'm so frustrated cause all the way there, the problem was more than evident.
