Same issue on my 2016 that's at the shop now getting sorted and the findings are,
Spotlight In-Operative = LED in passenger mirror
Night Vision In-Operative = Damaged front camera lens
My experience is exactly how you described.

I checked my camera and looks like it works normally, doesn't seemed damaged so it must be LED.
What do you mean by "LED in passenger mirror"? What is that?
Thanks!

The LED in the passenger side mirror aka (LED, Mirror Headlight, Puddle Light, Spot Light, ect . . .) Shop where you will - I did manage to get a part number
Puddle Lamp - Mercedes-Benz (099-906-44-02).
Again, the motor car is still at the dealer waiting for the camera to come from Germany and to have the same errors as you stated and have a bad camera is odd for me right now.
Please let us know when you replace the LED if the Spotlight In-Operative alarm clears.

I found out from the dealer service, spotlight is part of nightview. It flashes high beams when it see a object. Does not have nothing to do with passenger mirror light led. Mine has same error due to broken nightview camera lens

Spotlight Inoperative here in the UK on my owned from new W217. All searches on the Internet point to here. Obvs not a huge amount of chat about this,. I guess the average S Class owner has a driver, or doesn't need to fault find themselves! Hopefully I can add to stuff.
Same as other posters, a few seconds after start-up, I get the dread message " Spotlight System Inoperative". If I hit the night vision button on start-up I get a proper night vision image for a few seconds, before the error message.
It looks like I scored a direct hit to the camera from a stone...

This was back in 2022, and I then put some electrical tape over the camera. I still wasn't sure at this point if this was the night vision camera, ACC, or something else.
The night vision has worked OK until recently, until I started to get the error message. After picking up on this thread, I decided to look into it further, so took the bumper off and started to dismantle.
