This is what happened, so I am hoping you guys can chime in and let me know what to look at:
1 positive 1 negative for the front speakers
1 positive 1 negative for the rear speakers
1 positive 1 negative for the subwoofer
My system was working perfectly, I could adjust the bass of the in car speakers through the headunit, and adjust the sub through the sub control on the headunit.
A while back I decided to only take out my class d amp, cap and wavebox with sub and put it in our jeep cherokee. We just had the factory headunit in the Jeep, so the install shop set it up so that I can adjust both the in car speakers and sub woofer volume through factory head unit bass settings. There was no settings for the subwoofer volume in the Jeep headunit, so I was fine with that.
This weekend I put my class d amp, cap and wavebox back in my E55 since we sold the Jeep. This is where the problems lies as the 2 inputs behind the receiver for the subwoofer does not seem to be responding. The system has no problem playing all in car speakers, but when I plug in the subwoofer controls into the subwoofer inputs behind the receiver, the sub wont play. I then take out the subwoofer controls and put it in either the front or rear in car speaker inputs behind the receiver, it hits perfectly. BUT, I can only adjust subwoofer volume through moving the in car speakers bass volume like it was in the Jeep.
I am thinking the installer shop switched a setting in the class d amp so that it will only adjust subwoofer volume through the bass adjust when I had it in the Jeep.
Here is a pick of the back of my class D amp, and I have the subwoofer controls from the headunit plugged into the input L and R channel(not sure if it is on slave or master setting), subwoofer speaker wires in the center + - speaker output.

- I have a complete aftermarket sound system in my car been run by a 4 channel Power Acoustik BAMF 1200/4 channel amp.
- I also have a big wave box in my trunk running a 12" 1500watt rms sub powered by the Power Acoustik Class 1D 5500watt amp and digital farad cap.
- My upfront receiver is Alpine

Sounds like your head unit has the sub-pre amp outs turned off....
Try taking the Alpine sub out and plug it into the rear channel on your other amp and see if you get anything.

I agree, your issue sounds to be caused by the sub out rca from back of hu. Especially if it works plugging into left/right outputs. Either the sub out is turned off at the hu or you need a new hu

So must I make sure all my in car speakers are playing and the sub controls are plugged into the sub input behind the HU, unplug the control from the class d amp, and plug it into the rear input from the 4 channel amp?

Is the sub amp turning on? If not check your remote turn on wire. Also check and make sure the sub amp is turned to the master setting not the slave setting.

Make sure the rca's get plugged into the input on the amp from the sub out on your cd player. Its the first set of inputs(the top and bottom labeled L ch and R ch) on the far right of your amp in the picture you posted. Did you verify the amp was turning on when plugged into the the sub outs on the cd player? Also what model cd player do you have?
