I was always under the impression that with B&O I would have to live with the spare tire in the middle of the trunk cargo area. I am not wild about that restriction. However, my SA told me that 2014+ do not even have spare tires? They are all run flats? If this is true, then why do I see what appears to be a spare tire in the trunk of B&O cars for sale like shown below? Is this not a spare tire? Is having a spare now an option? A friend of mine with a 2015 C63 said he just has a kit with a compressor and gunk to patch the hole with no space saver.
Anyway, I just need to make sure I am making an informed decision when I pull the trigger on my next E63.

My 14 E63 does not have B&O but does have an inflatable tire in the trunk. Pretty sure all of them have a spare tire, although inflatable.
Note* the B&O cars do come with a spare, but it's in a separate bag. It does not fit under the trunk cover like the non B&O. It's separate and tied down on the trunk, taking up all the space. or just simply removed .. roadside assistance anyway so not 100% needed

Yes. This is what I have always understood. Basically the space where my spare sits under the trunk deck is used for the B&O Sub. And that is why the pic has a spare in a separate bag as you mentioned. I guess I am trying to understand why my very seasoned and knowledgeable SA at the dealer was saying they don't have spares in any of the 2014+ E class. Is he wrong?
I have heard that run-flats are just not that well of a performing tire, and they are pretty rough riding. Any of you guys with B&O use run-flats to work around this space limitation?

I have a 2014 with B&O AND an inflatable spare...and it fits under the carpet in the trunk (not in a separate bag). Furthermore, I do not have run flat tires (I have Pirelli P-Zero's).

Finally someone that has direct knowledge. Can you explain at all why the picture I put in the OP seems to show up in a lot of used cars with B&O? It sure looks a lot like a spare in a separate bag kinda strapped in place. Is there actually room under the carpet deck for both the B&O sub and the space saver? I wouldn't think so.
Any chance you can post a pic of your setup? As in lift the carpet deck and take a snapshot?
