Can you do the C63 style paddle shifters on this for direct bolton onto an 04 clk55?

Yes, we can provide the paddle shifter you are requesting:

The above pic is for SLK350 which has flat shifter but modified to longer paddle shifter. However, you need to return your paddle shifter since we need modify it with your shifter.

This process requires additional $350 for the paddle shifter from Mercedes.

Unfortunately, we dont have your model steering wheel in-stock. We can modify your steering wheel but it takes 4 weeks.

Do you know if this is as simple as changing out the wheels and plug-in-play, or do I need to adjust parameters in STAR to make the paddle shifters work?

THX. The steering wheel looks fantastic.

No adjustment needed on your part. To convert the flat tiptronic shifter into long paddle shifter, i need your shifter first since we need to take out the chip and place it into the paddle shifter. You will receive the steering wheel with the paddle shifter rest is plug and play. PM if you need further info.
