That brake cost seems rather high. Have you checked the price elsewhere? Brake pads and rotors aren't covered under the extended warranty so the fact that the car needs a brake service doesn't matter to your decision on the extended warranty.
You have a GLE 350 with the 4 cylinder engine that appears to be building a reputation for self destructing. Has Mercedes in Canada extended the warranty on that motor?

Take a high deductible.
I would not buy this or any warranty. In 2 years your out of pocket repair cost could be zero. Or it could be >$5k. It’s probably closer to zero.
Brakes are not part of warranty but rather normal wear items. DIY or indy for brakes.
The CAD$4800 would be USD$3360 for the ELW.
Using these rates:
1 USD = 1.43310 CAD
1 CAD = 0.697730 USD

That's what ELW's are - paying someone to share a portion of risk.
I'm OK with a $3-4,000 repair, but I pay to protect against a Big One.
Personal choice.
But my understanding is that there’s a big price variance in these EW products, depending on who is selling it. So shopping around will certainly help to ease the price of entry.
PS: I have never purchased EW for any of my prior products, and I tend to keep my cars for the long haul (200K and beyond). If problems are present, I would expect them to surface early on, especially problems related to the complex bits. I maintain my vehicles diligently, and reduce the manufacturer’s suggested maintenance interval to half of what they recommend (5K instead of 10K etc).

It's a personal matter of one's own risk tolerance.
Even if I have the resources to replace an engine, I don't have the desire.
Same with a possible injury or malady. I buy health insurance.
Personal choice. YMMV.
