2009 or 2010 ML350??
Are there any mechanical issues I should know about between the two? Anything else that stands out between either of them?

You can't go wrong with any of the two. In my case, it was a decision based on what was available at my price range, desired mileage (less than 35k), color, and premium features. Pay attention to premium features, as they differ btw the 2009 and 2010 model. For example, a 2009 P1 will not have power mirrors, but a 2010 P1 will.

I have a 2009 with P2, no leather, no airmatic, but I do have RSE (Rear Seat Entertainment) which I would have much rather had BiZenon lights. But such is the way things are buying used.

i guess I have P1 with MBTex vs Leather, no airmatic, no Distronic, no rear entertainment (that would be sweet if I did), no cooled seats, and I'm not positive but I don't believe it has Xenon headlamps either. The lowbeams look awfully yellowish when iluminated.
But other than that I love this car! I'm guessing MB pre-wired a lot of other features in there to make it easier to install later?
Now if only I could get my iphone to connect to the damn car's system.

So if W166 follows the same as W164 your W164 may or may not be prewired for RSE.
One day soon I need to call mBrace and see if someone there can teach me how to use RSE. Had no trouble registering my phones without a manual but even with the manual I can't figure out how to get past "Must enable from COMMAND" message on the rear when attempting to play a DVD in the rear player. OK, I get it that kids in the back can't watch DVD unless permitted from the front but I haven't found the permission button yet.
Also one of the headphones for the rear doesn't work. Have extended warranty but suspect headphones are exempt.
Most German designs with Bi-xenon headlights power on with he lights pointed down and the light moves up for the next second until reaching proper aim. HID/xenon lights flash bright the moment they start so its best they not be pointing at other drivers. Also real HID headlights have auto-leveling, once again to keep out of other driver's eyes no matter how your vehicle is loaded. Aftermarket retrofits do not.
The halogen lights on my W164 are better than the HID on my Prius.
