Removing W221 door-panels..
After the screws you need something thin, flat and wide and does not scratch paint to pry the panel off the door. It has pins all around the perimeter. Then you need to lift it up a bit but not before removing the tweeters.
I changed my door pins and have photos, just have not had chance to post a walk through. I will try to get something together in next day.

And also be carfeul when unclipping it, because there are many ribbon-wires that are easily damaged.

But I still have problem to remove the rear panels.. se below
You have to roll the window down because you need to grab underneath the panel top against the windows side and then pull carefully up
The Door Panel hanging on a hook it helps to press a little bit in top of panel when you remove the panel and when you put it back... see image

I will check that out when i come home from work today if i'm not finding it i will
attaching a picture that you can mark the screw on. If you can be so kind..
In worst case..If that is ok for you
I will post a pic that show where the screw is located
Finally ! mystery solved. Like Always.. easy when you know it

To get access to the last main panel screw You have to remove the speaker cover Please see pic's enclosed
If you dont need access to the el switch in the armrest. Leave it alone you don't need to loosen that one at all.

after thinking it over I think you can remove the panel without rolling the window down
Remember to remove the tweeter speaker first (new pic enclosed) then follow the pictures that explains the panel removal procedure better than word

When panel is loosened all the clips are out crab the back end upper level
i think you will be able to get your fingertips under there.(See new pic)
Then lift, rock carefully the panel up (It helps pressing the panel in a little bit in the middle with your other hand because of the hook the panel hangs on again see pic's)

You need to remove the speaker grill please see my pictures from today (-:
